Patients to familiarize themselves with the policies of their particular insurance plan regarding referrals, participating hospitals, labs, and amount of co-pay.
Contact member service department of your insurance company before services are rendered should you have questions regarding coverage or payment.
Notify our staff of any changes in personal or insurance related information at least 24 hours before your scheduled appointment
Present your insurance card at each visit so our staff can verify and maintain accurate information.
Payment to be made for co-pays and/or balances at the time of service. We follow the guidelines of all insurance companies for which we are contracted regarding co-pays for lab work

Payment is expected at the time of service unless other arrangements have been previously made.
